Safety Policy
It is our intent to provide a safe and healthy environment, Sapphire Counseling &Assessments asks that you please follow the following policies listed; No dangerous weapons on the property. No use of threatening or intimidating behaviors. No possession of alcohol, illegal drugs, Kratom, Delta 8 or 9 on the property. Medications must be within your possession at all times.
Payment Policy
Our services are billed directly through our office. Our providers participate in most of the major health insurance plans in the region. Please be aware that with any health plan, there may be deductibles. Please check with your insurance company prior to scheduling your session for specific information. In our office, payment is expected at the time of service. A valid credit card is requested to be on file for all clients. Any balances over $50 we reserve the right to run your card on file.
Weather Policy
If Lincoln Public Schools are closed due to inclement weather, our office will also be closed. Some providers may choose to still see clients through our online telehealth services so please check with your provider directly.
Cancellation Policy
We ask that all clients kindly notify us of an appointment cancellation at least 24 hours prior to your appointment or the last business day if scheduled on a Monday. If you are unable to give us 24 hours notice, your account will be charged $50. This fee is not covered by insurance and must be paid before your next scheduled appointment. This policy does not apply to Medicaid and Medicare clients.
HOLIDAY CANCELLATION POLICY
Due to high demand on identified holiday weeks, all missed or cancelled appointments will be charged the full fee for any appointments scheduled during those identified times. This will be for all scheduled appointments and will be identified 30 days ahead of time.
